About community hospitals

There are nine community hospitals and four 'Step-Up Step-Down" units across Cumbria providing a range of services which include inpatient facilities, day hospitals, minor injuries units and outpatient departments. 

No two hospitals are the same because of the history, geography and needs of the local population.

Inpatient care is provided to adults over the age of 16 years who are medically stable. Care is planned and delivered by nurses and therapists. Medical cover is provided by GPs.

The service is available for all eligible adults who require 24 hour care, along with therapeutic or nursing interventions for a short period of time. Through a model of enablement, patients will receive rehabilitation, nursing and appropriate medical based interventions.

Admission criteria

Patients with an exacerbation of a known chronic illness who require treatment or rehabilitation.

 

 

  •  Patients who require palliative or terminal care.
  • Patients who require a programme of rehabilitation.
  • Blood transfusion where cause of anaemia is known.
  • Patients with a condition not requiring District General Hospital care, but who require nursing, medical or therapeutic invention.
  • Intermittent or continuous intravenous or subcutaneous fluids.
  • Intravenous antibiotics in appropriate circumstances.
  • Patients who require a period of further medical / social assessment prior to determining long term care needs.
  • Patients who require step up – Step Up Step Down care.

Patients can be referred to a Community hospital from the following sources:

  • GPs (including GP out of hours)
  • Acute hospital trusts
  • Community nurses
  • Social workers
  • Palliative care services
  • Intermediate care services
  • Specialist teams and services

As part of the care process, a multi-disciplinary team meeting is held each week to determine and review the care pathway and discharge planning arrangements for each patient.

Case conferences may be held as required, involving Adult Social Care, General Practice, voluntary organisations and patients' families / carers.

The community hospitals are working more efficiently than ever, with increases in throughput and reduced length of stay. Nurses, doctors and associated health professionals are working closely together to enhance patient services.
